We’ve recently launched a Community Health Assessment (CHA) to better understand the health needs and priorities of residents across our region. The results will guide new programs, initiatives, and policy improvements that directly address the most pressing health challenges in our communities.
We’re asking for your help in spreading the word about this important survey. By sharing it with your networks, clients, and community members, you can help ensure every voice is heard — especially those who are often underrepresented.

Survey Details:
- What: A brief (10-minute), anonymous Community Health Assessment survey; open now through January 30, 2026 (end date subject to change).
- Why: To identify and address key health needs in Northern Bristol County
- How: Via QR code on the attached flyer or using the included paper survey
- Languages: Available in multiple languages
- Support: I’m happy to print and deliver paper surveys upon request, and will collect them at the end of the survey period
Participation is:
- Voluntary – participants can skip questions or stop at any time
- Anonymous – no identifying information will be collected
- Open to residents 18 years or older
